Signs of Magnesium Deficiency: A Pharmacist’s Guide

If I had a coin for every time a patient described the classic signs of magnesium deficiency without ever having heard of the mineral, I’d have a very heavy pocket.

Magnesium deficiency is surprisingly common, and it’s sneaky. Standard blood tests often miss it because less than 1% of your body’s magnesium lives in the blood. The rest is stored in bone and soft tissue. So you can walk around with a perfectly normal serum magnesium result while your cells are running on empty2. I’ve seen this more times than I can count, patients told everything looks fine, continuing to feel anything but.

These are the signs I’ve learned to listen for. Not as a diagnosis. I’m not your doctor. But as a pharmacist who has had this conversation hundreds of times, I know these patterns well enough to say: if several of these sound familiar, magnesium is worth a serious look.

Why magnesium deficiency is so easy to miss

Beyond the unreliable blood test, there are several reasons deficiency has become so common and so consistently overlooked. Modern agricultural soil is significantly more depleted than it was fifty years ago, which means the food grown in it contains less magnesium than it used to. Processed food strips what little remains1.

Chronic stress burns through magnesium stores faster than most people realise, and then compounds the problem by making sleep worse, which depletes it further. Certain medications accelerate the loss too. Diuretics and proton pump inhibitors are the ones I counsel patients about most often, both of which increase urinary magnesium excretion. High coffee and alcohol intake do the same.

The result is a population that is quietly, chronically short on one of the most important minerals in the body.

Who is most at risk

In my clinical experience, certain groups show up repeatedly. Pregnant women are at the top of the list. The demand for magnesium increases significantly during pregnancy, while nausea and dietary changes often reduce intake. Women with PCOS are similarly affected, particularly because insulin resistance accelerates magnesium loss.

Older adults absorb magnesium less efficiently from food and are more likely to be on medications that deplete it. Athletes and people under chronic stress lose more through sweat and cortisol-driven excretion. Anyone eating a highly processed diet with minimal vegetables, nuts or seeds is likely running low.

The most common signs of magnesium deficiency

1. Muscle cramps, twitches and restless legs

This is the one I hear most often. A calf that seizes up in the middle of the night. An eyelid that won’t stop twitching. Legs that feel like they need to move even when you’re exhausted4.

Magnesium is a natural muscle relaxant. It competes with calcium at the muscle fiber level. Calcium says contract, magnesium says relax. When magnesium runs low, calcium dominates and muscles stay in a semi-contracted, irritable state. At night, when you’re still and circulation slows, that twitching and cramping gets louder.

2. Fatigue that doesn’t make sense

Not the kind that comes from a late night. The kind where you’ve slept eight hours and still wake up feeling like you’re wading through water.

Magnesium is required to produce ATP, your body’s primary energy currency. Without enough of it, that ATP molecule stays inactive. Your cells are running on a low battery all day, even when everything else looks normal on paper.

3. Tired but wired at night

You lie down and your body is heavy but your brain is loud. That is a hallmark presentation of low magnesium and one I discussed in detail when explaining how magnesium activates GABA receptors.

Your brain’s calming pathway depends on magnesium to function properly. When levels drop, the brake pedal gets soft. Your mind races because the switch that tells your nervous system to power down isn’t being flipped the way it should be. Sleep becomes light, broken, or frustratingly out of reach.

4. Anxiety, irritability and a short fuse

This is the mental health presentation and it is the one most often dismissed or attributed to personality or circumstance. Low magnesium means your nervous system is idling at a higher baseline. Small things feel disproportionately large. Patience thins without obvious reason.

It is not a character flaw. It is neurochemistry. Magnesium helps regulate the hypothalamic-pituitary-adrenal axis, your body’s central stress response system. When magnesium is depleted, cortisol can spike and stay elevated. You feel on edge not because life has become harder, but because your body’s ability to return to calm has been compromised.

5. Heart palpitations or a skipping beat

This one can be frightening. You’re sitting still and your heart suddenly pounds, flutters, or seems to pause for a moment. Magnesium is essential for maintaining the electrical stability of the heart. It helps regulate the movement of potassium and calcium across cardiac cell membranes, keeping the rhythm steady. When magnesium is low, those membranes become more excitable. The result is extra beats, skipped beats, or a heart that feels like it’s doing something it shouldn’t.

If you experience palpitations, see your doctor. That is not negotiable. But I have seen patients whose palpitations resolved entirely once their magnesium status was properly corrected.

6. Frequent headaches or migraines

Not just any headache. The kind that builds behind one eye or tightens the whole skull in a band. Magnesium deficiency is well documented as a contributing factor in both tension headaches and migraines. The mechanism involves blood vessel constriction and altered neurotransmitter activity. Magnesium helps vessels relax and dampens the pain signals traveling through the trigeminal nerve3.

Some neurologists now use intravenous magnesium as an acute migraine treatment. That clinical reality speaks for itself.

7. Numbness and tingling

Pins and needles without an obvious cause. No awkward position, no compressed limb. In more significant magnesium deficiency, nerve conduction can be altered as calcium enters nerve cells without adequate counterbalance, causing them to fire erratically. This is less common than the other presentations but worth noting, particularly when it appears alongside several of the symptoms above.

8. Constipation

You may already know that magnesium citrate is used as a laxative because it draws water into the bowel. But adequate magnesium levels are also needed for normal smooth muscle contraction throughout the digestive tract. When magnesium is chronically low, things slow down in ways that are easy to miss as part of the bigger picture. What to do if this sounds familiar

My first advice is always the same: see your doctor. Symptoms like palpitations, persistent numbness and unexplained fatigue deserve a proper workup. Several serious conditions share these presentations and ruling them out matters.

If you’ve had a full evaluation and everything came back normal, or if the symptoms are mild and clustered around sleep, muscle tension and stress tolerance, magnesium is a reasonable and low-risk place to start. A standard blood test is not a reliable way to assess magnesium status, so a normal result does not rule out deficiency.

If you and your doctor decide supplementation is worth trying, the form you choose matters significantly. Some forms are well absorbed and gentle on the stomach.
